Scrub Sizing Guide for Bulk Orders: How Hospitals Get the Fit Right

Getting scrub sizing right for a bulk order takes three things working together: real employee body measurements, an approved finished-garment chart for the exact pattern being ordered, and physical fit samples that get worn and tested before anyone commits to a size mix. Chest or bust, waist, hip, height and inseam should be captured under one written method, then checked against movement and a laundering cycle before the order is placed. A size label alone is not a safe way to convert between suppliers. XS through 3XL can mean different finished dimensions depending on the pattern and the manufacturer behind it.

What's the difference between body measurements and garment measurements?

Body measurements describe the wearer. Finished-garment measurements describe the completed scrub, including the design ease and construction allowances built into it. Keep them in separate columns.

Body measurements are taken on the employee, around or along the body. Garment measurements come from the finished top or pant, typically laid flat. The two answer different questions, and a specification that blends them together invites confusion.

Ease is the gap between the body and the garment: the room built in for movement, layering and the intended silhouette. There's no single ease value that applies to every scrub, because it shifts with fabric stretch, cut, role and personal preference.

One common trap: a flat chest measurement on a garment is only half the circumference, measured side to side. Doubling it gets you closer to a comparable figure, but the result still includes ease and isn't the same as a body-chest circumference. Treating the two as interchangeable is where sizing programs go wrong early.

ISO 8559-1:2017 sets out anthropometric definitions for body measurement. ISO 8559-2:2025 separates body-based size designation from the garment dimensions a designer and manufacturer actually choose.

Which body measurements does a scrub program actually need?

Most scrub programs run on five consistent body dimensions: chest or bust, waist, hip, height and inseam.

The exact list depends on whether the order is unisex, men's or women's. Tops generally need chest or bust, waist and body height. Pants need waist, hip and inseam, with rise or thigh worth adding wherever fit problems tend to show up.

Use consistent working definitions across every measuring session:

Employees should wear light, non-bulky clothing and stand naturally, with the tape close but not compressing the body. Record whether each figure is in inches or centimeters, and never mix the two systems in one chart.

Are ASTM and ISO body-size tables enough on their own?

ASTM and ISO body tables are reference data. Every employer still needs a size chart built for the specific scrub pattern it's ordering.

ASTM D6240/D6240M-24a covers adult male chest sizes 34 through 52 across Short, Regular and Tall categories. ASTM is explicit that these body measurements support apparel development only alongside fabric, ease, styling and fit, not on their own.

ASTM D5585-21 covers adult female misses figure types in sizes 00 through 20, and ASTM maintains separate tables for other populations and figure types. Forcing every employee into one misses or male reference table because it's convenient is exactly the shortcut that produces a poor-fitting order.

These tables are design inputs, not a guarantee that any given manufacturer's medium matches the reference population. Workforce proportions, garment block, fabric and intended fit can all diverge from it.

The safer path: get the supplier's actual finished-garment chart, then confirm it with samples. That's what separates real sizing from a "standards-based" chart that never gets validated against the model you're buying.

How should finished scrub tops and pants be measured?

One approved point-of-measurement diagram should define every dimension, measurement path, unit and allowable production tolerance for the garment.

A spec that lists "top length" without saying where that measurement starts and ends invites inconsistency. Small differences in tape placement can make identical garments look like a fit problem.

Useful finished-top measurements include:

  • Half chest, shoulder width and center-back length
  • Sleeve length, armhole and bottom opening
  • Pocket width, depth and placement

Useful finished-pant measurements include relaxed and extended waistband, half hip, front rise, back rise, thigh, inseam and leg opening.

Every dimension needs a code, a written method and an illustration. State whether curved seams are measured along the seam line or straight across, and whether elastic measurements are relaxed, extended or both. Use one unit system throughout. ASTM D6240 notes that SI and inch-pound values aren't exact equivalents and shouldn't be combined.

How do you run a scrub fit event that actually works?

A fit event should offer the exact approved model, in enough sizes and lengths, for employees to test the movements their job actually requires.

Book a private, accessible fitting area and hand out written instructions. Samples need to be in the intended fabric and pattern, because stretch, weight and construction all change how a garment fits once it's on a body rather than a form.

Have employees test:

  1. Sitting, standing and walking
  2. Reaching overhead and across the body
  3. Bending, squatting and using loaded pockets

Check neckline coverage, shoulder mobility, sleeve position, waist security, rise, seat ease and pant length. The right size lets someone move through ordinary clinical tasks without extra fabric getting in the way.

Record top and pant sizes separately. A medium top and a large pant for the same employee is a normal outcome, not an error, when garments are issued individually. And push feedback past "too small" or "too large": ask what specifically felt wrong (shoulder restriction, waistband rolling), so you can tell a size-selection issue apart from a pattern defect.

Unisex, men's or women's scrub sizing: which should a hospital use?

A hospital can run multiple fit blocks at once, but each one needs to be treated as its own model, with its own chart and its own sample approval.

Unisex patterns simplify inventory. They don't guarantee a comfortable fit across every body shape. Men's and women's patterns improve fit choice at the cost of more models, more samples and more stock records to manage.

Compare the options using the same fabric and the same job movements. Don't assume a women's size sits a fixed number of steps away from a men's or unisex size. There's no universal conversion between them.

Give each fit block a stable model code in the purchase file. If two patterns differ in grading, pocket placement or proportions, list them separately even when they share a color and a visual style.

How should petite, tall and extended sizing be handled?

Length and extended-size needs belong in the first fit round, not tacked onto an order that's already been approved at standard sizes.

Petite and tall aren't just a shortened or lengthened hem. Rise, knee position, torso length, sleeve length and pocket placement may all need adjusting along with it.

Extended sizes should preserve the garment's intended movement and pocket function rather than scaling every dimension by the same flat percentage. Pattern grading decides how measurements shift between sizes, and different body areas often need different increments.

Build an accommodation route for employees who can't use the standard closures, cut or configuration. The EEOC's disability dress-code guidance (a US employment-law resource) describes a case where an employee couldn't use standard buttons and zippers, or wear the uniform comfortably while seated in a wheelchair. The employer and employee worked out an alternative uniform as a reasonable accommodation. Does stretch fabric fix sizing problems?

Stretch improves movement. It does not fix an unsuitable garment block, insufficient coverage or an inaccurate size breakdown.

A stretch percentage on a fiber label doesn't describe extension or recovery. Fabric can stretch in two directions or four, and it can behave differently after repeated laundering than it did fresh out of the bag.

A closely fitted stretch scrub can still pull at the armhole, ride up during bending or sag under loaded pockets. Sizing up too far solves none of that. It just trades one problem for excess fabric and unstable pocket placement.

Test stretch recovery by watching whether knees, elbows, waistband and pockets return toward their original shape after movement, then repeat the check after the garment has gone through the proposed laundering process. Why measure scrubs again after laundering?

Bulk buyers should compare at least two measurement sets: the approved garment before laundering, and the same marked dimensions after the specified care process.

A sample that fits correctly out of the box can change after a wash-and-dry cycle. Length, width, seam alignment and stretch recovery can all shift enough to affect which size an employee should actually be wearing.

Record the actual pre-care and post-care measurements rather than settling for "minimal shrinkage" as a description. The acceptance spec should name the test method, the care condition and the result both sides agreed to.

How do you build the final bulk size breakdown?

The final size mix should come from measured employee selections plus documented replenishment needs, not an equal split across every available size.

Build one row per employee or approved position, with separate columns for top model, top size, pant model, pant size and length. Once that's verified, consolidate the rows into an order matrix.

Skip the universal spare-stock percentage. Replenishment depends on hiring, turnover, replacement policy, supplier lead time and past issue records, so document the reason behind any quantity that goes above measured demand rather than defaulting to a round number.

Confirm that each color-model total clears the applicable MOQ. Sizes can normally be mixed within a 100-piece minimum as long as the color and model stay the same.
